At Etan's Journey, we offer a range of specialized services designed to meet the unique needs of vulnerable adults. Our 24/7 clinical and supportive care is focused on improving post-acute recovery outcomes, reducing hospital admissions, and promoting health stability.
24/7 clinical care
Our facility provides round-the-clock clinical supervision and care by Registered Nurses (RNs) and Licensed Practical Nurses (LPNs), ensuring continuous medical support for our residents.
Personalized supportive services
Certified Nursing Assistants (CNAs) offer essential daily living assistance, personal care, and emotional support, contributing to a comfortable and healing environment for every resident.
Comprehensive case management
Our team, including Licensed Clinical Social Workers (LCSWs) , Licensed Clinical Mental Health Counselors (LCMHCs), and casemanagers ,provide vital clincial care and case management to connect residents with community resources.
Post-acute recovery & stability
We focus on improving recovery outcomes, reducing hospital readmissions, and collaborating with partners to secure long-term housing and health stability for our medically fragile residents.
